  • Works with patients with addiction and dependence on opioids maintain or start medication assisted therapy using Suboxone
  • Provide case management for these patients both with physical and emotional needs
  • Performs initial assessment and intake obtaining social, medical, mental health, substance abuse and medication history.
  • Collaborates with MAT treatment care team as needed and for all scheduled appointments
  • Provide education and assessment to patients looking to access MAT treatment programs
  • Assess and monitor patients in the induction, stabilization, and maintenance phases of treatment in scheduled appointments or via telephone calls
  • Provide ongoing management, education, and support of patients in all phases of treatment. This includes scheduled appointments, routine labs, medication teaching, monitoring, film/pill counts, pending refills and prescriptions to providers

The Suboxone Bridging Program provides funds for suboxone patients who are experiencing a lapse in insurance coverage to cover the cost of their suboxone prescription. This ensures treatment continuity and helps prevent relapse.
  • Participates in quarterly Wadena and Ottertail County Opioid Taskforce meetings
  • Participates in quarterly Chemical Health and Multi Drug Prevention Coalition (CHAMP) of Wadena County

  • Collaborated with Steve Rummler Hope Network to secure 40 kits of nasal Narcan to distribute to Astera patients who would need to cover the co-pay for this medication
