· Plan, coordinate, and oversee the care of patients on peritoneal dialysis and home hemodialysis. Manage home therapies at two dialysis units consecutively. Educate patients on how to do their dialysis at home. Ongoing care of each patient to ensure treatment stays on track. Draw monthly labs and administer medications per protocol. Manage monthly clinic visits in person and via Telehealth. Evaluate patients’ ability to perform dialysis treatments and take all prescribed medications. Review patients’ lab work, home medications, and dialysis therapy. Inform providers about changes in patients’ conditions. On call two weeks per month, answering patient’s phone calls related to dialysis therapy. Perform home visits to home dialysis patient on regular basis. Ensure patients’ follow up with transplant center. Collaborate with social worker, dietician, and physician to manage each patients’ specialized needs. Consistently achieved metrics in the top 10% of the company.

· Coordinate and organize the daily flow of the heart and vascular department. Supervise stress tests, and administer IV’s. Update and implement protocols. Provide education to patients and staff, and telephone triage. Float congestive heart failure nurse, monitoring patients’ daily weights virtually, and dosing diuretics off protocols. Resource to nurses and other support staff.

· Sole nurse to 4 providers in nephrology clinic while being available to neighboring cardiology clinic. Job duties include rooming patients, data collection, calling patients with results and instructions per provider’s orders. Assist patient with any questions they have.
