Complete comprehensive start-of-care, resumption of care, recertification, transfer, discharge and other required assessments;evaluate clinical, functional, psychosocial, care-giver and home environment needs.
Develop, implement, review and revise an individualized plan of care in collaboration with the patient, caregiver, provider and interdisciplinary team.
Provide skilled nursing services within the RN scope of practice, which may include assessments, medication management and teaching, wound care, catheter care, disease process education, IV related care, and monitoring for complications.
Obtain, clarify, implement and communicate Physician/Provider orders; promptly report significant changes in condition, adverse findings, hospitalizations and care plan updates.
Coordinated services with PT, OT, ST, medical social work, home health aides, pharmacists, DME vendors and community resources to achieve optimal patient outcomes.
Maintained accurate, timely clinical documentation including assessments, visit notes, care coordination, medical reconciliation, care-plan updates and discharge documentation to support continuity of care.
