Provide care to veteran's in their homes based on provider orders and HBPC Plan of Care

Set up, provide education and reconcile medications

Observe, maintain and promote skin integrity

Obtain and record vital signs

Identify and prioritize veteran care needs, including emergent and non-emergent situations in the home

Administer vaccinations and injections

Recognize, report and document changes in veteran's condition to the provider and RN Case Manager

Participate in IDT

Administered prescribed medications in accordance with VA HCS policy and procedure

Assist veterans safely at meals times. Measure and record intake

Provide safe positioning, ambulating and transferring with proper equipment

Bladder scan and straight catheterization

Administer respiratory therapy such as oxygen and nebulizer treatments

Observe, maintain and promote skin integrity

Document routine data such as treatments, ADL's, vital signs and restorative care

Observe, chart and report any changes in veteran's condition to RN

Communicate appropriately with veteran's, families and team members

Blood glucose checks and monitoring
